Vendor's submitting invoices to the Court that have both goods and services can download and use the Vendor Invoice/Pricing Template (Form FN-049) if the purchase involves materials, labor and freight.

The taxable and non-taxable items must be itemized for proper reporting of sales tax and use tax to the Franchise Tax Board.

Note: failure to use the template when submitting your invoices can cause delays in the Court providing a timely payment for your invoices.
